Form Selection Note
Please use this form when submitting membership applications or renewals for multiple students.
If you are submitting a membership or membership renewal for only one student please use the Student Membership Application/Renewal Form
- Download the Multiple Student Entry Form and enter the Student information as directed in the spreadsheet.
- Once you have completed and saved the Multiple Student Entry Form – Complete the Advisor Information area below.
- Upload the completed Multiple Student Entry Form using the Upload Button below.
- Enter the Total Due amount from the Multiple Student Entry Form in the Total Amount Due field below .
- Submit this form.
When you select the Submit button, you will be transferred to PayPal where you will be able to pay with your PayPal account or pay using a Visa, MasterCard or Discover card (this option is below the PayPal login).
